About director of nursing (don) roles

A director of nursing (DON) leads an organization’s nursing team and is accountable for the quality and compliance of the care it delivers. In home health, the DON (often called the clinical manager) supervises clinicians, oversees documentation and regulatory compliance, and prepares the agency for surveys.

What a home health DON does

  • Supervises RNs, LVNs, therapists and aides
  • Oversees admissions, care plans and visit documentation
  • Leads quality assessment and performance improvement (QAPI)
  • Keeps the agency ready for state and accreditation surveys
  • Handles clinical escalations and takes part in on-call

Regulation and accreditation

Medicare-certified home health agencies follow the CMS Conditions of Participation and submit OASIS (Outcome and Assessment Information Set) assessments. In Texas, home health agencies are licensed by the Health and Human Services Commission as Home and Community Support Services Agencies (HCSSAs). Many agencies are accredited by an organization such as CHAP (Community Health Accreditation Partner), ACHC or The Joint Commission.

The Outcome and Assessment Information Set: the standardized patient assessment that Medicare-certified home health agencies complete at set points in care.

Is a home health DON the same as a clinical manager?

Usually. Medicare’s home health rules use the term clinical manager; many agencies call the role director of nursing.
